Impact Transition Services
Impact Transition Services delivers structured, safe and supportive transition services for people leaving custody. Our work focuses on the critical point of release, offering transportation, guidance and stabilisation support to help reduce risk and improve reintegration outcomes.
Initially supporting young people and, in time, adults, we work with individuals who may be at risk of homelessness, disengagement, reoffending or social isolation following release. Through early intervention, we help people access accommodation, support networks and essential services, while building confidence and stability during the transition back into the community.
We work in partnership with justice services, local authorities, community organisations and support providers to create effective pathways that support successful resettlement and contribute to safer communities.
