To provide practical and emotional support to young carers and their families based on their individual needs, to ensure that as far as possible their caring responsibilities do not prevent the young people reaching their full potential.
Main duties and responsibilities
To facilitate a young carers assessment and support plan
To work to contract requirements
To identify, engage and support hidden young carers
To raise awareness of young carers needs within appropriate settings
To work as part of a busy young carers team; ensuring a time limited but robust support service is delivered
To offer information, advice and guidance to service users and their families
To raise awareness of young carers issues amongst professionals and the public to enable them to be supported in their caring role
Advocate for young carers
Where Humankind cannot solely meet identified needs of a service user, when appropriate signpost and/or act as a Lead Professional through the Early Help Assessment and co– ordinate multi agency support
To provide support in a variety of settings including home visits
To support with the planning and execution of activities and support groups for young carers and their families
To be prepared to work creatively and flexibly in line with the needs of the service, this may include evening and weekends
To ensure case notes are recorded within company guidelines
To ensure effective case management
To provide written and verbal reports as required
To ensure that all project recording and information systems are adhered to and kept in place, utilising an online case management system
To ensure risk assessments are carried out and regularly reviewed
To establish and work within existing networks, attending relevant meetings and groups and agencies across the geographical areas, and represent Humankind as appropriate
To maintain an awareness of relevant legislation, practice and research as required within the service delivery unit
To report all problems to the Project Leader/Project Manager as soon as possible
To work as part of a team, attend staff meetings, participate in staff training and development, supervision sessions and participate in the activity of the service delivery unit
To work within Humankind’s Charter of Values and Policies
To contribute to the development of the project
To be involved in Humankind’s working parties
To meet targets set by Project Leader/Project Manager
To implement Quality Systems relevant to the funding streams
To work to Humankind’s Child Protection and Vulnerable Adults policy and procedures and local safeguarding board arrangements.

Person Specification - Essential criteria:
Experience of completing comprehensive assessments and action plans with service users
Experience of taking on the lead professional role when multi agency support is required by a service user
Communicate confidently and effectively
Knowledge of safeguarding children legislation and current national strategies
Good report writing and I.T. skills
To carry out duties with regard to a commitment towards principles of equality and diversity
Have a planned and organised approach to working
Able to work as part of a team and also on own initiative and have the ability to stay calm under pressure
Demonstrate good self-awareness, appropriate use of supervision, appraisal and self-development
Ability to work in a multi-agency setting, with other partner agencies, demonstrating strong liaison skills
Have experience of working towards and achieving targets
Be able to work autonomously
Person Specification - Desirable criteria:
Experience of planning and delivering group activities/ support sessions
Knowledge of benefit entitlement and housing issues would be beneficial
